Medication synchronisation service

It is not unusual for medications to get out of synchronisation due to a change in your treatment, hospital stay or even a holiday.

The practice policy allows us to issue your medication up to 2 weeks before it is due.

Recent feedback from our patient participation group has highlighted the frustration in having to return to the pharmacy or practice multiple times to order medication that is due at different times.

To help with this we offer a medication synchronisation service, please download and complete the form below, drop it into reception or email harcourt.surgery@nhs.net and we will do our best to issue a interim supply to pull everything back into line.
